• GotSport Log-In Details for your team account
• 2024/2025 Qualifying League
• 2024 NYSW State Cup Result
• 2024 Qualifying League Result
• Cell Phone & Email address of primary contact & coach
• Credit Card for payment - the application fee is $595.00 and due at the time of application
